PERSIST Scholars Program Application Forms
Each applicant should download and complete the appropriate application form. Other supplemental materials required with the application are listed. The application forms are provided here both as Microsoft Word documents and as PDF files; you may use either one. Be sure to select the appropriate application form for you.
In-coming Freshman:
1. Complete on-line Intent to Apply Form.
2. Download application form:

3. Mail completed application and proof of financial need (choose one):
- Abbot District Resident
- Copy of EFC estimate from College Board
- Copy of EFC from FAFSA
Transfer Student with Associate's Degree from a Community College:
1. Complete on-line Intent to Apply Form.
2. Download application form: Word Document or PDF file
3. Mail completed application, unofficial college transcript, and proof of financial need (choose one):
- Abbot District Resident
- Copy of EFC estimate from College Board
- Copy of EFC from FAFSA
