Beneficiary Designation

It is very important to keep your beneficiaries updated.   This is especially true if you have any life altering events like marriage, divorce, new child or death in your family.   Members of PERS, TPAF, PFRS and ABP must use this form to change their beneficiary information for their life insurance. In addition, members in ABP must also complete a beneficiary change form with their respective vendor.   If you would like to change your beneficiary with the Division of Pension and Benefits, please click on this link http://www.state.nj.us/treasury/pensions/beneapp.pdf

Make a copy of this form and sent to the Division of Pension and Benefits.
