Please evaluate yourself and each of the other members of your design project team/work group by rating each person on the five criteria listed below. Your evaluations are confidential and will not be shared with other students. Be honest and fair in your assessment of others and yourself.

Complete this form and return it no later than 1 week after the seminar period when groups give their presentations. The form can be copied to your clipboard and pasted in a word processor, completed and given to me or pasted into the text block of your e-mail and sent electronically. It may also be printed out directly from this web page.
