HTH Student Insurance Form

MADRID participants do NOT need to complete this form

Please submit this form in order to ensure that you will be covered during your travels abroad. The HTH Student Insurance is included within the total cost of the tour. This insurance will cover you for emergency medical transportation, sickness & hospital benefits, accidental death & dismemberment, baggage and travel delay, repatriation of remains (up to a set amount), and much more.

Include:

• a copy of your TCNJ student ID
• a copy of a secondary form of ID (driver's license, etc.)

Form to be filled out by your physician or by a physician at TCNJ Health Services. If you wish to make an appointment at Health Services, call them EARLY in order to secure an appointment at:

(Have this form filled out as soon as possible, preferably anywhere from 1-6 months before you leave, since appointments fill up near the end of the school year)
