Requesting Your Medical Records
Medical records contain important information about an individual's medical history. A record is started at Health Services for each student enrolled at The College of New Jersey (beginning with the student's "Medical Information Form"). Notes are made in your chart each time you receive services at Health Services to document the visit and provide a comprehensive "record" of your health care experiences, which is important for your care in the future.
Your Health Services' medical record is strictly confidential. This record is not released to anyone without your written consent, except in compliance with state and federal laws (i.e., subpoena, court order, health department reporting requirements, etc.). Health Services maintains all patient medical records for at least 10 years after you leave TCNJ.
To obtain or release copies of your
medical record, complete and bring or mail a written
request to Health Services. You may either use our Authorization
to Release Health Information Form
or if you have a problem viewing this online form, you may simply
submit a letter to us. We cannot accept verbal or email requests. This letter must include the following information:
- Your full name
- Your full name while attending TCNJ (if different)
- Your TCNJ Student ID number (we realize that not all students have this number - in this case, please provide the last 4 digits of your social security number and make sure to record your date of birth where requested.)
- Your date of birth
- The date you entered and exited TCNJ (if appropriate)
- The specific information you would like released; you must also include a time frame (example: Sept. 2004 to May 2005) and the type of information (example: immunization record; TB test record)
- Your signature and the date (we cannot legally process those requests that are not signed)
There is a $5 flat fee for this service, payable to The College of New Jersey Health Services by certified check, personal check, cash or money order. Requests for records will NOT be sent out until payment is received and until your personal check clears (approximately 3-5 business days). There is no charge if the record is being sent to your personal health care provider.

Records will be faxed, mailed or ready for pick up as soon as possible, usually within 3-5 business days. Students picking up their records in-person must bring photo identification with them.
Want your record faster but can't come in person to Health Services?
You've waited until the last minute and now you need it "yesterday". You can now pay the medical record processing fee online with Discover, MasterCard or American Express (Visa not accepted). Go to www.tcnj.edu/~stuacct - Click on "Bill Payment Options" - Click on "Credit Card Payment Site" - Scroll down to "Medical Records". Type in $5.00, then press TAB on your keyboard (there is a $.12 charge for this convenience) - Type in your credit card information. Then click on "Submit". Once your payment clears (usually 24 hours - business day), we will send out your record. Archived records are kept off-campus and may take 2 business days to retrieve. Call first to discuss your record request with a Health Services' office staff member (609-771-2483).
Requesting Records from Psychological Counseling or TCNJ Planned Parenthood Office:
To request your record from TCNJ Psychological Counseling Services, call 609-771-2247.
To request your health record from the Planned Parenthood TCNJ Office, contact Planned Parenthood at TCNJ at 609-771-2110. Please note that Planned Parenthood is open only during the Fall and Spring semesters. They are closed during semester breaks and in the summer. If you need your health record and Planned Parenthood is closed, please call the Planned Parenthood main office at 437 East State Street, Trenton, N.J. at 609-599-4881.
