The following is a sample of the student evaluation form my students filled out based on my performance as a junior practicum teacher on my second-to-last day.  In bold under each criteria I placed my average rating in each capacity of the 33 student responses.  Since I never experienced a fire drill or assembly with my students, there was insufficient data to compile an average response for number 12.
