Title	Description
Application Fee*	$300 - Payable in Student Accounts (Green Hall 119) ONLY. This fee is non-refundable. Application fees paid to program sponsors are in addition to this application fee paid to TCNJ. Please note:* TCNJ waives the program application fee for students going on study abroad programs through NJSCIS.
Application Packet	Includes: 2 Reference Forms Emergency Treatment Permission & Contact Form Academic Evaluation Form Essay Assumption of Risk & Release Form Disciplinary Verification Form (instructions on how to fill out all forms)
PROGRAM APPLICATION FORMS	Program specific application forms. Click on the link to find your specific program application form and supplemental materials required.
Transcript	Log onto PAWS, go to "Academic Records" and click on "Request Official Transcript" and have it sent (there is no fee) to Green Hall 111. *FOR TRANSFER STUDENTS ONLY: In addition to your TCNJ transcript, please also request transcripts from all previous institutions you have attended. Have these transcripts sent to Green Hall 111.
Medical Exam ISA MEDICAL EXAM FORM*	Available On-Line DUE TO HIGH DEMAND, AS OF SEPT. 21, 2009, TCNJ HEALTH SERVICES IS NO LONGER PROVIDING PHYSICAL EXAMS UNTIL FURTHER NOTICE. Please get you Medical Exam completed by your personal physician. Form to be filled out by your personal physician or by a practicioner at TCNJ Health Services. TCNJ Health Services will provide a physical free of charge even if you do not receive Student Health Insurance. Please make an appointment for your medical exam at least 1 MONTH* in advance of the deadline date.* To make an appointment: Eickhoff Hall, Rm 107 P) 609.771.2483 F) 609.637.5131 E) hlthserv@tcnj.edu For students applying for ISA program ONLY -* You must submit the ISA Medical Exam IN ADDITION to the TCNJ Medical Exam Form. BRING BOTH forms with you to your appointment.
Program Price Sheet	Be sure to ask for a price sheet for your program and the estimated additional costs such as books and personal expenses. The Office of International & Off-Campus Programs (Green Hall, Room 111) will have program price sheets and estimated expenses for all but a few programs.
Program Cost &Financial Aid Form	Available On-Line To be filled out if you recieve or wish to recieve financial aid to cover the whole or partial cost of your program. The "Program Cost & Financial Aid Form" is used for TCNJ office purposes only. You DO NOT need to send this to your school as part of your application and IS NOT required by the deadline.
